    Through the consular department you can apply for the following certificates:

    1.    Registry summary (personal status, country of birth)

    Please note:
    Birth certificates are the only certificates that can be requested in both English and Hebrew – a dual language certificate.
    For the certificate in Hebrew, please fill in the form in Hebrew.
    For the certificate in English, please fill in the form in English (in capital letters)
    The rest of the certificates are ONLY in Hebrew. If you need a translation, you must use an official translator.

    Birth certificate:
    1.    You must fill out the request form. Please click on this link.
    2.    Ordering a birth certificate incurs no costs. If you need the birth certificate with an apostil stamp, For costs, please click here.


    Death certificate / Registry summary:
    You must fill out the correct form:
    This service is done at no cost. If you need the document with an apostil stamp, for a fee, please click here​.

    4.    Exits and entry certificates (to and from Israel)

          Certificate of citizenship or non-citizenship:
    This form requires arrival in person to the embassy, or arrival of a power of attorney

          Israeli Police clearance:
    You must fill in the form requesting the police clearance​ and declaration certificate​.
